questionnaire form that will inform us of your specific foot conditions
and custom orthotic needs.
- Take a mold of your feet, fill out the questionnaire form, and ship your foot impressions with the questionnaire form to us.
- We will design, fabricate, and ship your
custom molded orthotics to you within 7 to 10 business days from the
time we receive your foot impressions and questionnaire.
